How to Become a Patient

Qualifications:

  • Bradley County Residents (Dental and Medical), Polk County Residents (MEDICAL ONLY)

  • Ages 19 or older

  • Uninsured or under-insured

  • Total HOUSEHOLD income at 150% or below the federal poverty level

Financial Screenings

Drop by for a financial screening at our clinics to see if you qualify.

Screening times are Monday - Friday from 9-11 am and 1:30-2:30 pm. Please bring the following items with you:

  • Proof of HOUSEHOLD income for ALL people working and/or receiving income in the house (pay stubs, SSI letters, bank statements, child support letter) If you are not working, please bring signed letters of support from those who support you.

  • Photo ID

  • Current lease, bill, or piece of mail with your name and address (to prove you are a Bradley/Polk County resident)

  • Insurance cards (if you have any)

  • Medication list

  • $25 cash to cover the first appointment
